Chapter 3 - The Unraveling Syndicate

The fluorescent lights of the ICU room cast a stark, clinical sheen over Evelyn’s pale face.
The mechanical hum of the ventilator echoed with every breath forced into her lungs. Her fingers lay motionless against the white sheets, bruised and yellowed from the endless IV lines delivering pressors, antibiotics, and blood thinners into her system.
Claire sat beside the bed, holding her mother’s limp hand. For two days, Evelyn had remained in a medically induced coma while her liver and kidneys slowly worked to process the synthetic toxins that had built up in her bloodstream.
Dr. Thorne and Detective Miller stood near the doorway, speaking in hushed, urgent tones.
"We ran the toxicology report on the remaining vials we found in Evelyn’s home," Dr. Thorne said, handing a digital tablet to the detective. "It’s horrifying. The injections weren't prenatal vitamins or standard progesterone. They were heavy doses of synthetic estrogen mixed with an experimental immunosuppressant called Cyclo-D9—a drug banned by the FDA eight years ago."
"Why give her an immunosuppressant?" Miller asked, scanning the lab results.
"To stop her body's immune system from attacking the misplanted tissue," Thorne explained grimly. "An abdominal pregnancy triggers a massive inflammatory response. Her body was trying to reject the ectopic tissue from day one. The clinic used Cyclo-D9 to suppress her natural defense mechanisms, forcing her body to retain fluid and swell, mimicking a normal third-trimester pregnancy. They created a fake bump, gave her fake ultrasound scans via an encrypted app, and kept charging her credit cards five thousand dollars a month for 'remote monitoring.'"
"It’s extortion disguised as miracle medicine," Miller growled, his face hardening. "They knew she was dying. They were just milking her bank account until her heart gave out."
"Did you trace the local contact?" Thorne asked.
"That's the dangerous part," Miller replied, lowering his voice even further. "Evelyn didn't get all those injections overseas. Our surveillance team reviewed the security footage from Evelyn’s neighborhood in Columbus. Every two weeks, an unmarked white sedan pulled up to her driveway. A man wearing home-nurse scrubs went inside for twenty minutes."
Claire turned her head from the bed, her eyes wide with fear. "Someone was coming to her house?"
"Yes, Miss Vance," Miller said, walking closer to the bed. "A local courier was delivering and administering the Cyclo-D9 injections right here in Ohio. We cross-referenced the license plate of that sedan. It’s registered to a shell company affiliated with a disgraced former obstetrician named Dr. Victor Vance—no relation to you—who lost his medical license in Illinois five years ago for running illegal stem-cell trials."
Suddenly, a faint, metallic clicking sound came from the heart monitor beside Evelyn’s bed.
Evelyn’s eyelids fluttered. A low, raspy groan escaped her dry lips beneath the tape securing the ventilator tube.
"Mom!" Claire dropped to her knees beside the bed, her hands grasping Evelyn’s shoulders. "Doctor! She's waking up!"
Dr. Thorne rushed to the side of the bed, immediately checking Evelyn’s pupillary responses with a penlight while the ICU nurse prepared to reduce the sedative drip.
"Evelyn, can you hear me?" Dr. Thorne asked clearly, tapping her shoulder gently. "You are in the hospital. You are safe. Do not try to speak—you have a breathing tube in your throat."
Evelyn’s eyes squeezed shut, tears leaking from the corners, tracing deep channels down her hollow cheeks. She looked up at Claire, her eyes filled with a raw, terrifying terror.
She began to shake her head violently against the pillow, her breath catching against the plastic tube, causing the ventilator alarms to sound a high-pitched alert.
"Easy, Evelyn, easy," Thorne urged, placing a hand on her chest. "Nurse, give her ten milligrams of IV push lorazepam. She's panicking."
Evelyn’s hands twitched against the sheets, her fingers clawing at Claire’s wrist with surprising strength. She raised two fingers, tapping desperately against Claire’s palm, making a gesture like writing with a pen.
"She wants to write something," Claire said frantically. "Give me a pad!"
Detective Miller handed Claire his notepad and a ballpoint pen. Claire slipped the pen into her mother’s trembling fingers and held the notepad steady over her chest.
Evelyn’s hand moved erratically, the blue ink forming jagged, chaotic letters across the white paper:
H-E I-S C-O-M-I-N-G
Claire frowned, reading the words aloud. "He is coming? Mom, who is coming?"
Evelyn pressed the pen down so hard the paper tore:
T-O-N-I-G-H-T. T-H-E N-U-R-S-E. H-E S-A-I-D I-F I T-A-L-K-E-D H-E W-O-U-L-D K-I-L-L C-L-A-I-R-E.
A cold, paralyzing silence fell over the room.
Detective Miller’s hand instantly dropped to the butt of his sidearm. "Miss Vance... did the man who gave you the injections threaten your daughter?"
Evelyn nodded furiously, tears raining down her face as her pulse monitor spiked to dangerous levels again.
"He told her..." Claire whispered, her voice shaking violently as she realized the horror her mother had been living in for seven months. "He told her that if she went to a local hospital or stopped taking the injections... he would hurt me."
"He was controlling her through fear," Dr. Thorne said, his voice laced with pure rage. "She wasn't just a victim of medical fraud. She was a hostage in her own home."
Detective Miller immediately unclipped his radio from his shoulder. "Control, this is Unit 4. I need an armed perimeter guard on ICU Room 312 immediately. Set up a secure checkpoint at the West Elevator bank. We have a active threat against a primary witness."
Miller turned back to Evelyn, his face grim. "Evelyn, listen to me very carefully. The man who gave you those injections... does he know you were brought to this hospital?"
Evelyn closed her eyes and shook her head slowly, then opened them and tapped the notepad again.
