Chapter 5 - The Confession in the ClinicThe medical wing on the lower level of the North Estate was clinical, sterile, and entirely insulated from the rest of the fortress. The hum of an air-purification system provided the only sound as Adrian’s polished shoes clicked against the white epoxy flooring.

Dr. Elena Vance was standing at a stainless-steel counter, methodically organizing surgical instruments into a tray. She wore her crisp white lab coat, her silver-streaked hair tied back in a neat bun.

She didn't turn around when the heavy door clicked open. She must have known by the cadence of the footsteps alone who had entered.

"Her blood pressure should stabilize by tomorrow morning," Elena said calmly, her back still turned. "As long as she avoids further emotional shock. Pregnancy at thirty-four, especially with twins—"

"Twins?" Adrian stopped dead in his tracks.

Elena paused, her hands freezing over the steel tray. She slowly turned around, her professional composure slipping just enough to reveal a flicker of unease in her eyes.

"Oh," Elena murmured, pressing her lips together. "You didn't know."

Adrian closed the distance between them in three long strides, his dark eyes boring into hers. "You said twins."

"Yes," Elena said, recovering her composure and lifting her chin. "Maya’s medical chart from her OB-GYN in Atlanta listed a twin gestation. Two healthy girls. I assumed she told you when you met in the office today."

Adrian stared at her. The world seemed to tilt on its axis for a second time that day. Not just one daughter. Two. Two little girls with Maya’s spirit and, God willing, his stubborn refusal to break.

Then the anger returned, burning hot and fierce beneath his skin.

"Why?" Adrian asked, his voice dangerously low.

Elena blinked. "Why what?"

"Why sell my family to Marcus Vance?" Adrian stepped closer, towering over her. "Twelve years of loyalty. Twelve years of sitting at my table, treating my people, knowing damn well what happens to traitors in this house. And you sold my wife’s location for a bank wire?"

Elena didn't cower. She let out a dry, humorless laugh, leaning back against the metal counter.

"Loyalty?" she sneered. "To a monster who built an empire on blood and ruined thousands of lives? You think I care about your little criminal syndicate, Adrian? My brother is a monster, yes, but you aren't much better. You locked Maya in a cage, suffocated her until she fled for her life, and then acted like a grieving widower while you ran your underground empire."

"That doesn't answer the question," Adrian growled.

"I didn't sell her location to kill her," Elena snapped back, her eyes flashing with sudden, bitter defiance. "I gave Leo Vance her location so they would force her out into the open—so you would finally have to face what your life does to the people you claim to love! I wanted you to wake up! I wanted you to realize that as long as you wear the crown of the Chicago underworld, everyone you touch is a target!"

Adrian stared at her. There was no remorse in her eyes—only a twisted, self-righteous justification.

"You risked the lives of my unborn children to teach me a lesson?" Adrian’s voice was barely a whisper.

"I risked nothing that wasn't already burning," Elena said coldly.

Adrian didn't yell. He didn't strike her. He simply reached out, his hand moving with terrifying speed, and pressed a heavy panic button mounted beneath the counter—the direct line to the estate’s maximum-security detention block.

The heavy steel door of the clinic clicked open instantly, and two armed guards stepped inside.

"Take her down to cell block B," Adrian said without looking at the guards. "Strip her credentials, freeze every asset she owns, and make sure she never practices medicine again. If she breathes a word of what she knows about my family outside these walls, feed her to the river."

"You're a tyrant, Adrian Cole!" Elena shouted as the guards grabbed her arms and dragged her toward the door, her white lab coat fluttering behind her. "You'll never change! Even with twins, your world will destroy them!"

The heavy steel door slammed shut, cutting off her voice entirely.

Adrian stood alone in the sterile clinic. He reached up, rubbing the bridge of his nose as a dull, throbbing headache blossomed behind his eyes.

Twins.

And he had almost lost them because his world was built on violence, betrayal, and blood.
